Bachelor’s Degree preferred.

Medicaid Waiver (HCS) experience REQUIRED

Current Nursing License in state of employment.

Minimum 12 months experience as an RN.

Ability to establish a comfortable and supportive relationship with a person receiving supports

Ability to take instructions and constructive criticism

Valid Driver’s License


Participate in the admission of new clients selecting ABLE FAMILY SERVICES as provider.

Participate in SPT, IP, and IPC meetings,

Delegate staff as needed including day program staff

Supervise LVN

Ensure that adequate medical records and a current list of medications by physician are in chart before completing a CNA

Develop implementation plans to guarantee that the medical and healthcare needs of the clients are met and ensures the appropriate medical follow-up.

Ensure that justifications for the IP are accurate and fits the need of the client and client care.

Coordinate annual nursing assessment with client and HHCC care staff.

Provide special instructions to client, HHCC staff and support staff related to medical diagnosis, allergies, diet, etc., and is available for ongoing consultation to staff regarding medical problems and healthcare.

Clarify medications and update documentation to reflect current medications.

Gather health information from client and foster care staff quarterly and annually for assigned participants.

Provide documentation regarding individual’s health status for various support people.

Make referrals for needed medical services and ensure that follow-up care is pursued.

Working Conditions:

May be On-Call 24 hours a day for medical emergencies, in cooperation with other management staff.

Work will take place in the administrative office, community, and in the homes.

$60,000 -70,000 per year
